The purpose of this form is to bring before the Secretary of the Navy annually, through the Bureau of Yards and Docks and other bureaus concerned and the budget officer of the Navy Department, the estimated needs of the shore establishments in the way of public works improvements. The procedure is described in Chapter 8 of this manual. This form is accompanied by instructions in detail for its preparation, which instructions may vary from year to year and should be carefully examined. Original complete, and two copies of certain parts, to the Bureau of Yards and Docks, through official channels; additional copies as may be required locally. Size of sheet, 8 by 10 inches.

Submitted upon installing new power plant equipment.

Navy yards, naval stations, and shore establishments
of all classes having heating, lighting, or power
plants.

The purpose of this report is to provide and maintain at the Bureau of Yards and Docks, and locally, a continuous record of heating, lighting, and power plant equipment. Instructions are printed on the form. When equipment is removed from a plant, report is to be made to the bureau by letter or survey, whereupon the fact is recorded upon the report on this form on file in the bureau. Original to the Bureau of Yards and Docks, through official channels; additional copies as required locally. Size of sheet, 8 by 10 inches.

Y & D form 86. Report of Progress, Public Works Contract.
Submitted monthly.

Public works officer or other officer in charge of public works
contract work.

All shore establishments having public works contract work in progress.

The purpose of this form is to keep the Bureau of Yards and Docks informed as to the progress of work under public works contracts, and to constitute a permanent record of such progress. Report shows status of work on contract, including plans approved, materials delivered, work in place, money expended, and time elapsed; delays, if any, and cause. Original and one copy to the Bureau of Yards and Docks direct; additional copies as required locally. Size of sheet, 5 by 8 inches.

Y & D form 87. Report of Progress of Work by Yard Force.
Submitted monthly.

Public works officer or other officer in charge of yard force jobs under special allotments.

All shore establishments expending funds under special allotments from the Bureau of Yards and Docks.

The purpose of this form is to keep the Bureau of Yards and Docks informed as to the progress of work under yard force jobs being performed under special allotments made by the bureau, and to constitute a permanent record of such progress. This report is not required upon jobs charged to the regular allotments made for general maintenance purposes, except where specifically so directed. Report states progress made on jobs performed by yard force, giving percentages of material drawn, construction work in place, and money expended for month and to date. Original and one copy to Bureau of Yards and Docks direct; additional copies as required locally. Size of sheet, 5 by 8 inches.

Y & D form 90. Boiler Report.

Submitted when boilers are installed.

Public works officer.

Navy yards, naval stations, and shore establishments of all classes having heating, lighting, or power plants. The purpose of this report is to provide and maintain at the bureau, and locally, a .continuous record of boilers ashore. The form contains spaces for entering the detailed characteristics of the boiler and its various parts. When boilers are removed from a plant report should be made to the bureau by letter or survey report, whereupon the fact is recorded upon the report on this form on file in the bureau. Original to the Bureau of Yards and Docks through official channels; additional copies as required locally. Size of sheet, 8 by 10 inches.

Y & D form 91. Quarterly Report of Steam Generators.

Submitted quarterly.

Officer in charge of power plant operation.

All shore establishments using steam generators.

The purpose of this form is to furnish the bureau with a quarterly report, giving data on tests, hours in service, pressures, condition of boilers, settings, stokers, piping and boiler fittings; also cost of repairs made and estimated cost of recommended repairs. Original to Bureau of Yards and Docks; copy to other bureau concerned, if any; additional copies as required locally. Size of sheet, 8 by 104 inches (4 pages).

Y & D form 96. Report on Electric Truck.

Submitted quarterly.

All naval shore establishments.

Report gives complete information on each electric truck.

Statement is made of

mileage, operating cost, and condition in detail for each vehicle. Original to Bureau

Plants.

Submitted monthly.

Officer in charge of power plant operation.

All shore establishments having central power plants.

The purpose of this form is to furnish the bureau with a monthly report affording a complete summary of the operation of power plants, giving quantities of all kinds of power generated, and cost; also stating all combustible or other materials used in generation, including oils for lubrication, and cost. Original to Bureau of Yards and Docks; copy to other bureau concerned, if any; additional copies as required locally. Size of sheet, 8 by 10 inches.

Y & D form 109. Report of Quarters Occupied or Vacated.

Submitted when officers' quarters are occupied or vacated.
Public works officer.

Navy yards, air stations, training stations, submarine
bases, and other stations, excepting ordnance sta-
tions, radio stations, and hospitals.

The purpose of this form is to enable the Bureau of Yards and Docks to maintain a continuous record of the occupation of officers' quarters. The form is self-explanatory. Original to Bureau of Yards and Docks direct (post card); additional copies as required locally. Size of sheet, 4 by 5 inches.

Submitted upon completion of buildings.

Public works officer or other officer in charge of construction of buildings.

Naval shore establishments.

The purpose of this form is to maintain in the Bureau of Yards and Docks, and locally, essential data concerning each building constructed. Report gives data relative to completed structures, material of foundation, superstructure, roof, dimensions, cubical space and floor space, cost, deterioration, and present value. Original to the Bureau of Yards and Docks direct; additional copies as required locally. Size of sheet, 5 by 8 inches.

Y & D form 124. Report on Service of Automobiles and Motor Trucks. Submitted semiannually.

Public works officer or transportation officer.

All shore establishments and vessels having automobiles, motor trucks, or ambulances.

The purpose of this report is to maintain in the Bureau of Yards and Docks, and locally, a record of the service of all Navy-owned automobiles (including busses), motor trucks, and ambulances (exclusive of Marine Corps vehicles), wherever located

and without regard to assignment or the bureau having control thereof. This record is maintained in this bureau under instructions from the Assistant Secretary of the Navy to facilitate coordination in the service. The form is self-explanatory. Original to Bureau of Yards and Docks through official channels; copy to other bureau concerned, if required by such bureau; additional copies as required locally. Size of sheet, 8 by 10 inches.

Y & D form 130. Real Estate Acquired, Occupied, Vacated, or Released.

Submitted upon acquiring, leasing, occupying, vacating, or releasing real estate.

Entire naval shore establishment.

The purpose of this report is to enable the Bureau of Yards and Docks to maintain the list of real estate and fixed improvement's required by article 426–6, Navy Regulations, 1920. Report should be made on all properties, without regard to their character or to the bureau or activity having control thereof, including properties under the control of the Marine Corps, and without reference to the conditions under which the property is or was acquired or occupied. Original to the Bureau of Yards and Docks through official channels; copy to other bureau or office concerned, if required by such bureau or office; additional copies as required locally. Y & D form 131. Mooring Report.

Submitted when a mooring is placed or materially changed.
Public works officer.

All shore establishments having offshore moorings. This report is for the purpose of providing and maintaining a record in the Bureau of Yards and Docks of moorings in place at the various shore establishments having offshore anchorage space. When a mooring is removed, the fact should be reported by letter or survey report and noted on the record form. The public works officer shall certify annually in the report of expenditures and operations, Y & D form 31, that all mooring reports have been rendered. Original to the Bureau of Yards and Docks through official channels; additional copies as required locally. Size of sheet, 8 by 10 inches.

Y & D form 132. Assignment of Motor Vehicles.

Submitted upon change of assignment of motor vehicles.
Public works officer or transportation officer.

All shore establishments and vessels having automo-
biles, motor trucks, or ambulances.

The purpose of this report is to enable the Bureau to maintain a correct list of Navy owned motor vehicles, showing the status and assignment of each, to facilitate coordination. The report applies to automobiles, including busses, motor trucks of all kinds, ambulances, and motor cycles, but not to industrial trucks for inside use. A change from "in storage" to "in service," or from one activity to another, should be reported,

but not mere temporary assignments. The Bureau is required to keep a complete list of all such naval vehicles without regard to which bureau or office has control thereof. Y&D form 135. Report of Shop Inspection of Structural Steelwork. Submitted monthly or upon completion.

The purpose of this report is to furnish the bureau with a report in detail on the inspection of material and fabrication. Original to Bureau of Yards and Docks, copy to inspector's file and to public works officer at yard where steel is to be erected. Size of sheet, 8 by 10 inches.

Y & D form 136. Report of Shop Inspection of Machinery.
Submitted monthly and upon completion.
Inspector of engineering material.

All machinery specified as requiring inspection at
factory.

The purpose of this report is to furnish the bureau a report in detail on the progress and processes of manufacture. Original to Bureau of Yards and Docks, copies to yard concerned and inspector's file. Size of sheet, 8 by 104 inches.

Y & D form 137. Expenditures, Passenger-Carrying Vehicles.
Submitted quarterly for each vehicle.

Public works officer or transportation officer.

All shore establishments and vessels having passengercarrying motor-propelled vehicles.

The purpose of this form is to enable the Bureau of Yards and Docks to maintain a record of expenditures under the limit fixed by law upon expenditures for the maintenance, operation, and repair of passenger-carrying motor-propelled vehicles as required by instruction of the Assistant Secretary of the Navy. Every item of expense must be reported without regard to the appropriation involved. Original to Bureau of Yards and Docks through official channels; additional copies as required locally. Y & D form 140. Pile Driving Record.

Submitted upon completion of each pile driving job.

All shore establishments.

The purpose of this form is to provide information for the use of the Bureau of Yards and Docks and of the yards and stations concerning all piles driven, either under contract or by day labor force. Each form submitted must show number for each pile, and shall be accompanied by a copy of a plan showing the location of each pile driven, each numbered as on the form.
